Step 2:

Take your completed signed application form with your payment and any proof  required to any @ctive card issuing site. Young people under the age of 16 will need the signature of their parent or guardian.

Proof of name and address

We will accept any of the following: driving license, council tax bill or book, gas/electric/telephone bill etc., bank statement, Post Office savings book, a family allowance book, medical card or a rental agreement
 
Please note if your benefit is a 'joint' claim and your partner's surname is different, you must show evidence of sharing the same address e.g. a letter addressed to both of you.
 

Proof of age

Only if applying for a senior citizen (aged 60 and over), student (16 years and over) or junior (under 16 years) card.
