Take your completed signed application form with your payment and any proof required to any @ctive card issuing site. Young people under the age of 16 will need the signature of their parent or guardian. |

Proof of name and address
We will accept any of the following: driving license, council
tax bill or book, gas/electric/telephone bill etc., bank statement,
Post Office savings book, a family allowance book, medical card or
a rental agreement
Please note if your benefit is a 'joint' claim and
your partner's surname is different, you must show evidence of
sharing the same address e.g. a letter addressed to both of
you.
Proof of age
Only if applying for a senior citizen (aged 60 and over),
student (16 years and over) or junior (under 16 years) card.
