This is a universal form which can be used by any elector to
apply to vote by post, either at a single election, or
indefinitely.

Application for a proxy to vote by post
|
|
This form enables a person, who has been appointed to vote as
proxy for another elector, to apply to vote by post. This will
normally be because the proxy does not live within the Peterborough
City area, but the form may also be used for short-term reasons,
e.g. the proxy will be away from home on the day of the
election.
