
What is transition?
Transition means moving from one
situation to another. It is a part of life. All young
people go through a transition as they prepare to leave school and
move into adult life.
For young people with special
educational needs and/or disabilities moving on from school can
bring its own challenges. Adulthood means not just leaving
school but also managing changes in health services, social care
support, benefits and funding, social activities and living
arrangements.

young people. It explains what it involves, things to think
about and where to go for more information.
