
Transition is where you change from one situation to
another
If you are a teenager you will soon be leaving school and
entering adult life.
Making plans concerning this is on-going and
continues whilst you are in education and learning.
There will be a transitional review meeting in
Year 9 with you, a teacher, and your Connexions Adviser. Your
parents orcarers will also be invited, and there may be others
there as well. There will be annual reviews in
Years 10 and 11 to update any plans that you have made.

Where are you now
What SUBJECTS do you like in school? This might help you think
about what to do in the future.
What HOBBIES and INTERESTS do you have outside of school?
Do you get EXTRA HELP in school that you will continue to need
after Year 11.
What about the future?
Do you still want to stay on at SCHOOL or go to COLLEGE.
What course are you interested in?
Would you prefer to get a JOB instead? Do your FRIENDS or FAMILY
work in a job you would like to do.
Do you need to find out about any MONEY you may receive or get
paid?
What are you able to do now for yourself, and what do you need
to learn to help you become more INDEPENDENT?
Are there things STOPPING YOU GETTING TO WHERE YOU WANT TO BE?
Your Connexions Adviser or teachers may be able to help.
Do you have HEALTH issues that may continue to affect you?
Does someone look after you? Will you still need to be CARED FOR
in the future?
There may be OTHER THINGS you wish to talk about at your
TRANSITIONAL REVIEW. Your CONNEXIONS ADVISER will see you before
your review, so you can talk to him/her first if you want to.
