Our offer:

  • Support to register for health services or doctor (GP).
  • A health passport when you leave care – this is your health history including a record of your immunisations, dental and other medical information.
  • Financial support for the cost of prescriptions. If required your Personal Adviser will include this in your Pathway Plan.
  • Priority access to Sexual Health Services.
  • Gym Pass for Vivacity gyms or leisure venues via your Personal Adviser
  • Good quality bicycle, helmet, and lock.

Help to understand your history

Children’s Services must keep records of your care history for 75 years. When you are 18 you are entitled to access your records (Subject Access Request) and can make this request (you must do this in writing) through your Personal Adviser – they will provide you with the support you need, should you decide you want to do this.

Your Personal Adviser will help you understand your rights, help you access information about you and provide support if you need to make a complaint.

We know that looking at records can be hard - it can be very upsetting and confusing to go through the information about your life and we strongly encourage you not to do this on your own but to have support – this can be your Personal Adviser, or you can choose to do it with a friend or both – whatever works best for you.

We know how important it is for this to be done in a respectful and supported way.
